Academy Students
If you are a student that was accepted into an academy here at Lakeland High School you should have received a letter in the mail claiming you have been accepted. At first it will appear that there is a sense of urgency for you to rush down to LHS and enroll. This is partially true. Academy students fall into two categories. Students are either an academy student on a transfer or not. If you are originally zoned for Lakeland High School than all you need to do is double check to make sure you are enrolled in the first level class of your academy. If you are not zoned for Lakeland High School than all you have to do is make a visit to the Hollingsworth Rd. office (Guidance Office) and enroll for LHS. Make sure you look below to see exactly what you need to enroll at LHS. Congratulations on your acceptance and welcome aboard!!!

Students entering Florida school for the first time

1. Birth Certificate (Certified with seal from vital statistics)
2. Evidence of physical examination within the last 12 months
3. Florida certificate of immunization (BLUE CARD) from the Health Dept.
4. Social Security Card
5. Two proofs of residence from TWO different categories
   1. Rent Lease or Mortgage document or Property Tax Record
   2. Current utility bill (within the last 30 days) must show parent name and address
   3. Voter registration
   4. Proof of Government benefits
6. Withdrawal form from last school or transcript of grades from last attended school
7. Name and address of last school with phone and fax numbers to order records

Students enter in from out of state
BUT HAVE BEEN IN A FLORIDA SCHOOL

1. Birth Certificate ( Certified with seal from vital statistics )
2. Florida certificate of immunization (BLUE CARD)
3. Social Security Card
4. Two proofs of residence (see #5 above)
5. Withdrawal or transcripts of grades from last attended school Name, address and phone number of previous school (Fax number if possible)

Students entering Polk County schools from another Florida County

1. Birth Certificate ( Certified with seal from vital statistics )
2. Florida certificate of immunization (BLUE CARD)
3. Social Security Card
4. Two proofs of residence (see #5 above)
5. Withdrawal or transcripts of grades from last attended school Name, address and phone number of previous school (Fax number if possible)

Students entering from another Polk County public School

1. Social Security Card
2. Two proofs of residence (see #5 above)
3. Withdrawal or transcripts of grades from last attended school Name, address and phone number of previous school (Fax number if possible)
