Lakeland High School Athletics, Participation Information:

·       Participants must have a physical each school year. Validity period starts June 1. A student may not participate in any activity without a physical and the necessary paperwork turned into the specific coach.

 ·       Participants must also sign the Release and Consent Form before they can practice or play with a team.

 ·       The Polk County School Board provides Secondary Insurance coverage to all athletes.  Your carrier serves as the Primary Insurance.

 ·       Students may participate in summer programs only at the school they will be attending.
